This form allows Veterinary Surgical Associates Client's to switch from the standard urn to any of the ones listed below at no additional cost.  By filling out the form AMS will be able to identify you and change the urn for your pet to one that you would prefer.  The default is the maple urn with your pet's name laser engraved on a plaque on the front of the urn. 

IMPORTANT - In order for AMS to correctly match you with your pet you must fill out the form completely by identifying your pet name and using the drop down boxes to let us know the type of pet and the clinic where your pet was picked up.   If the pet name you submit is different than the one provided by the clinic we will use the one submitted in this form.
